Completed all sidequests, collected all minerals, Asari writings, Salarian and Turian insignia, explored every planet (or almost every, as I remember).

Note: This is an old character I couldn't add until now because of the broken submission page, so I don't remember very well some details. I think she is a Sentinel and she reached level 60, but I'm not 100% sure.
Completionist, as far as I remember. Well, maybe almost completionist. Usually I stop searching minerals when my journal says I completed this quest.
BDTS installed; I saved the hostages.
Pinnacle Station: installed, but never completed (what a pile of utter crap, I add. I regret having paid for this DLC: it's rubbish.)
Conrad Verner: I met him. Due to the bug in savegame transfer between ME and ME2, it doesn't matter what I told him.
Garrus: recruited after talking with Harkin.
Wrex: recruited.
Chorban's assignment: completed, 21 keepers scanned.
Asari diplomacy: I accepted the assignment from Nassana, killed her sister and returned to tell her.
That pesky reporter at C-Sec headquarters: I punched her. This matters, because it affects the dialog with her in ME2.
Fist: killed (by Wrex, as far as I remember).
Helena Blake: I persuaded her to disband her gang.
Nirali Bhatia's corpse: returned to her husband.
Garrus pushed to Paragon status.
Sha'ira: I completed her quest, got that trinket and used it on Eletania.
Dr. Michel: I convinced the blackmailer to leave her alone.
Old friends: I met Finch and reported him.
Dead scientists: I convinced Toombs to leave Dr. Waine go.
Cerberus: I did Hades' Dogs, but I don't remember to whom I gave the files.
UNC-Hostages: the biotics surrendered (but this flag is buggy too).
NOVERIA:
- I got Anoleis arrested;
- I spared the Rachni queen;

- I convinced Jeong (Paragon way);

In terms of Udina/Anderson, I left the decision for the politicians (you can choose which you want in ME2).
Completed as most missions as possible, including those that were both Paragon or Renegade specific.

Treated Conrad Verner neutral at first, then nicely.
Killed Nassana's sister (UNC: Asari Diplomacy) before meeting with her, then used all renegade dialog.
Feros: Helped all colonists and did not kill any of them. Let the Asari help the colonists.
Novaria: Helped Parasini arrest Anolias.
Did Helena Blake's quest, and then let her live (with a warning).
Wrex killed Fist.
Nirali Bhatia -- Used all renegade dialog to intimidate the Alliance to return his wife's body.
Gave Tali the geth data.
Gave Wrex his family armor.
Encouraged Garrus on a renegade path.
Romance: Encouraged a Kaidan/Shepard/Liara love triangle, then chose Liara at the jealousy scene.
"Mixed" explanation: Ended the game with 100% Renegade and about 80% Paragon. Most major game choices were done Paragon, most side missions and dialog were done renegade. Used only intimidation and never charm. Encouraged crew (such as Garrus) on a renegade path whenever possible, but was also nice to all crew members.
Finished the game with the "rich" achievement, and at level 60. Game should import into ME2 with both Paragon and Renegade bonuses.
